The following forms are the most common forms that are used by vendors and departments. If you need something that is not listed below, please contact the Purchasing Department and we will be happy to assist you.


  • City of Westfield Purchasing Business Manual - All departments are encouraged to read this.
  • Vendor Addition Form - When filling out this document please list the department that is requesting the vendor addition.
  • W9 - This form must accompany the above vendor addition form and must be sent to the Purchasing Department.
